Position: Casual LPN
Description:
Supporting our interdisciplinary team of Palliative Care professionals, the LPN will focus on direct patient care including the administration of prescribed medication(s), following detailed patient care-plans and creating a compassionate and comforting care environment. LPN’s are required to complete all necessary patient care charts and documents and contribute to the overall cleanliness and upkeep of the clinical environment. Bobby’s hospice is open to friends and family and supported by numerous volunteers, therefore the successful candidate must be able to work well with others and capable of building strong relationships.
Key Responsibilities
- Provide comprehensive daily patient care in accordance with established standards
- Administer medications safely and accurately
- Transcribe and implement physician orders
- Ensure adherence to all organizational policies, procedures, and clinical protocols
- Promote and maintain a safe environment for patients, staff, and visitors
Qualifications:
- Trained and Certified RN/LPN available for days, nights, etc.
- Willing to expand your palliative care education
- Ability to work in a fast-paced, changing environment
- Exceptional communication skills and professionalism
- Familiarity working in an interdisciplinary environment
- Knowledge of the Not-for-Profit sector
- Able to stand for extended periods of time and work under stressful conditions
- Able to display compassion, empathy and be task-driven and focused
- Able to pivot and prioritize effectively
- Excellent organizational skills
Personal Support Worker (PSW)
Position Summary
Reporting to the Director of Nursing, the Personal Support Worker (PSW) works collaboratively with the interdisciplinary care team to provide compassionate, high-quality personal care and support to patients and families at Bobby's Hospice while maintaining a safe, comfortable, and respectful environment.
Key Responsibilities
- Provide personal care in accordance with individualized care plans.
- Assist patients with activities of daily living, including bathing, grooming, dressing, feeding, toileting, mobility, positioning, skin care, and comfort measures.
- Observe, document, and report changes in patient condition to the nursing team.
- Follow infection prevention and control practices and participate in Health & Safety initiatives.
- Support patients and families during end-of-life care.
- Maintain patient confidentiality and provide compassionate, respectful, person-centered care.
- Communicate and work effectively with patients, families, volunteers, and members of the interdisciplinary team.
- Demonstrate empathy, professionalism, and sound judgment when responding to challenging or sensitive situations.
- Prioritize multiple tasks while maintaining quality of care.
- Participate in continuing education, quality improvement initiatives, and Hospice activities as appropriate.
- Follow Hospice policies, procedures, and quality standards.
- Complete other duties as assigned.
Qualifications
Education & Experience
- Graduate of an accredited Personal Support Worker (PSW) program or equivalent.
- Knowledge of palliative care principles and personal care practices.
- Minimum one year of related experience preferred.
- Criminal record check and satisfactory references required.
Language: English required; bilingualism is considered an asset.
